How do I qualify for the statement credit offer?*

To qualify for a $50 or $100 statement credit*, you must:

1. Apply & open a new CareCredit credit card account;
2. Use your new card to pay for purchase(s) totaling $300 or more to earn a $50 statement credit or $1000 or more to earn a $100 statement credit; and
3. Register at carecredit.com/promotions with promo code OFFER2024 and complete the registration form.

All steps must be completed between May 1 - July 31, 2024.

How do I register for this offer?

• Online: Go to carecredit.com/promotions and enter promo code OFFER2024. You will then need to complete the registration form.
• Last 4 digits of CareCredit account number
• Valid Email Address
• Primary cardholder’s first and last name
• Primary cardholder’s address (should match address on account)

If qualified, you will receive statement credit within 3 billing cycles from date the promotion ends.

I have not received my statement credit. When will I receive it?

Please allow up to 3 billing cycles from offer end date to receive the statement credit. You are eligible for the statement credit after you have completed all the steps listed above between May 1 - July 31, 2024.

What is the last date to register for this offer?

You must register and complete all of the steps listed above by July 31, 2024, at 11:59 p.m. EST.

What is the last date to make a purchase (transact) to qualify for this offer?

Do I have to make the qualifying purchases at the same practice where I applied?

No. Any purchases with your new CareCredit card that add up to $300 or more will be applied toward qualifying for this offer.

Does the transaction need to be one purchase of $300 or more or can I make multiple transactions that equal $300 or more?

Either way is fine. As long as your purchases total $300 or more during the offer period they will qualify.
