From May 1 – August 31, 2024, become a new CareCredit cardholder
and complete the steps below to earn a statement credit:

1: Open a new CareCredit credit
card account.
Apply online, call 800-677-0718,** or scan
the QR code in our office.

| 2: Spend to earn a statement credit.
|
|
Use your new card to pay for purchase(s) totaling $300 or
more to earn a $50 statement credit or $1,000 or more to earn
a $100 statement credit.

| 3: Register for the offer online.
|
|
Go to carecredit.com/promotions, enter promo code
OFFER2024 and complete the registration form.
|
|
|
Cardholders will receive the statement credit within 3 billing cycles from date the promotion ends. All steps must be completed by August 31, 2024.
